Risk Register Template
A ready-to-use Excel risk register that scores each risk automatically and shows at a glance which ones are critical.
Description
Most organisations keep a risk register in a spreadsheet somebody built once. Scores are calculated differently by different people, the risk levels mean nothing consistent, and the rows below the first few quietly stop working.
This template supplies the structure and does the scoring. You enter a probability and an impact from one to five; the workbook calculates the score and assigns the risk level itself. The four levels are coloured, so a Critical row is visible without reading the sheet.
It comes with its own instructions sheet and a reference sheet stating exactly how the scoring works — the score is probability multiplied by impact, and the four bands are a stated design choice, labelled inside the workbook as not a regulatory requirement.
One clearly marked example row shows the intended use, and five further rows are pre-formatted with live formulas so the register works from the first entry.
Who it is for
The person responsible for keeping a risk register and without a method for it — an operations or quality manager, a project lead, a small-organisation risk owner.
No risk-management qualification is assumed.
The problem addressed
Risks are recorded inconsistently or not at all. Scoring differs between people and between periods, so registers cannot be compared, and a register that under-reports its own risk profile is worse than none because it looks authoritative.
The outcome
A register in which every risk is scored the same way, the levels mean the same thing, and the critical rows are visible immediately.
